Pharmacists Part of Prescription for Medicare
As Republicans and Democrats square off on Medicare, it is important that all parties remember the goal central to the debate: better health outcomes for seniors. Policy-makers are seeking to achieve this goal through a new prescription drug program, which Americans hope will improve access for Medicare recipients while also controlling costs. There is an important, often-overlooked participant on the front lines of the drug delivery system whose contribution has significant influence on that chief goal: community retail pharmacists.

Medicare Part D Information
Medicare Part D is offered to anyone with Medicare, whether you get your health insurance through the Original Medicare Plan or if you have a Medicare Advantage Plan (like an HMO or PPO). Part D is simply a prescription drug coverage that may help to lower the price of prescription drug expenses and to protect the costs from going up in the future. Medicare Part D providers are privately owned companies that are approved by Medicare. It is your decision to enroll in Medicare Part D. Normally you will pay a monthly premium in addition to your Medicare cost. This amount can be withdrawn from your Social Security check or your checking or savings account for added convenience. Some Medicare Advantage Plans may include this fee in their plan already with no added expense to you. If you join Medicare Part D as soon as you are eligible for Medicare, your monthly premiums will be the lowest possible price. If you do not enroll right away and you have went 63 days or more without prescription drug coverage, you will have to pay a penalty now and for the remainder of the time you are enrolled. Choosing the plan that is right for you can be as easy as visiting www.medicare.gov and choosing “Compare Medicare Prescription Drug Plans” or by calling 1-800-633-4227. Extra help is available for those who had incomes of less than $14,700 if you are single or $19,800 if you are married for the year 2006. You will have to provide proof of your income and show your assets, as there are limitations as to how much recourses you can have and still qualify. If you qualify, you will receive help paying your premiums, deductibles, and co-payments. You may apply for extra help by visiting www.socialsecurity.gov on the web or by calling 1-800-772-1213. After applying, they will mail you a letter letting you know if you qualify and what steps to take next. Some Medicare Part D plans may or may not have a deductible. A deductible is the amount of money you are required to spend out of your own pocket before the Original Medicare Plan, your prescription drug plan, or any other insurance will begin to pay your costs for healthcare and prescription drug coverage. Part D Prescription Drug Plans will have a list of medicines that are covered under their plan. This list is called a formulary, which includes many brand name and generic drugs. The formulary will contain information on how much your prescriptions will cost. Generally prescription drugs are grouped into tiers depending on the type of drug and given one price for the whole tier. The formulary may be updated regularly, removing or adding certain drugs depending on their availability. Most plans will notify you in the event a drug you use is removed from their formulary. Medicare Part D providers contract with certain pharmacies where you may purchase your prescription drugs. They have agreed to accept a lower price than normal for their medicines. These are considered in-network pharmacies. By having your prescriptions filled at in-network pharmacies you are guaranteed to get your prescriptions at your formulary price. If you receive extra help, co-payments for prescription drugs will be no more than $5 each. Pharmacies that are not in the provider’s network are considered out-of-network pharmacies. Except for a few reasons, if you have your prescriptions filled at an out-of-network pharmacy you will most likely have to pay for the full cost of your drugs. Some exceptions to this rule are if your medicine is not available in a timely manner at the in-network pharmacies or if you are traveling out of town. The plan you choose may have a coverage gap that is commonly known as the “donut hole.” After you have spent a certain amount of money (no more than $3850) on prescription drugs that are covered in your plan, including what plan providers have paid, you will have to pay any prescription drug cost that is necessary, until you reach a maximum of $3,051.25 spent out of your own pocket. Catastrophic coverage will begin once you have reached your plan’s out-of-pocket limit. Now you will only pay a small percentage, in the form of either coinsurance or a co-payment, for your prescription drug costs for the rest of the year. You must continue to pay any premiums to your plan, in addition to the cost of your medicine, during this “donut hole” or coverage gap period. If you receive extra help to pay for your drug costs, there will be no coverage gap. Understanding Medicare Part D and the terms that are used to describe the different plans will help you to decide if it is right for you.

Is anybody listening? Humana announces doubling of profits From Medicare Part D as seniors in Doughnut Hole pay full drug prices
While many seniors are stuck in the middle of the doughnut hole without Medicare drug coverage, paying their monthly premiums and 100% of the cost of drugs, Humana announces its profits more than doubled from its Medicare Part D plans. The Association of Community Pharmacists Congressional Network (ACP-CN) is troubled that Congress continues to sit back and watch pharmacy benefit managers (PBMs) and private health insurers, like Humana, continue to run away with taxpayers' money at the expense of real savings to America's seniors.

Health Blog : Drug Study: Medicare Benefit Works, Industry Says
The pharmaceutical industry has long been a fan of the Medicare drug benefit, spending oodles on lobbying to support its enactment and on awareness of the program since then. Now PhRMA, the industry's main trade group, is out with a report that finds Medicare Part D is working really well. Patients with no drug coverage in 2005 significantly reduced their out-of-pocket costs by signing up, the report says. Total monthly costs for these people, even with increased use of medicines, fell by 45% after they were covered. The report has less information on so-called dual-eligible people - those ...
